Photo and Video Release Waiver

I, [Parent/Guardian's Full Name] (printed below), hereby grant permission to TigerShark Aquatics, LLC to use photographs and/or video recordings of my child(ren) (as listed below), taken during any TigerShark Aquatics, LLC programs for promotional and marketing purposes on social media platforms.

I understand and agree to the following terms:

1. Use of Media: TigerShark Aquatics, LLC is authorized to use, reproduce, and distribute the photographs and/or video recordings in any and all forms of media, including but not limited to, social media platforms (e.g., Facebook, Instagram, TigerShark Aquatics, LLC website).

2. No Compensation: I understand that my child will not receive any compensation for the use of these photographs and/or video recordings.

3. Release of Liability: I release TigerShark Aquatics, LLC, its agents, employees, and any third parties acting under its authority, from any claims, damages, or liability arising out of or related to the use of the photographs and/or video recordings.

4. Duration of Consent: This release is valid indefinitely from the date of signing.

5. Withdrawal of Consent: I understand that I may withdraw this consent at any time by providing written notice to TigerShark Aquatics, LLC. Any use of media prior to the withdrawal will remain valid.

 

I have read and understand the terms of this release and voluntarily agree to its provisions.
